Child Care Network LLC approached us requesting a replacement for a cumbersome paper application.  Child Care Network LLC is a professional nanny agency that provides pre-screened nannies for families within South Eastern Wisconsin. The application contained five pages of questions used to perform background checks, assess prior employment and educational background, and to contact references. It is a critical piece within the placement process.

After some discussion we were able to work out a solution that fit within the budget available. We agreed to develop an electronic form that would allow Child Care Network LLC to receive form responses via email, and could then forward the applications to potential clients for review.

Form construction and revisions were completed within approximately 2 weeks.

Features/Development Notes:

- The application was developed using PHP which allowed us to continue using the current web host.

- We were able to use the shared SSL certificate provided by the host to keep costs down.

- The responses are sent in a formatted HTML email, allowing for printing of the application.

- All responses are saved and encrypted. This allows for retrieval in the event that the email is lost.
